Position Description

Bilingual Hospice Social Worker - Part Time - Merrimack Valley-328938
Description
The Hospice Social Worker is the member of the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient / family unit, define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.
This position will be a 20-hour per week opportunity, covering the Merrimack Valley area of MA.
Primary responsibilities include:
  • Visiting patients to assess care and provide pain/symptom control
  • Participating in care conferences and working in interdisciplinary teams
  • Supporting the patients' families with funeral arrangements, psychosocial needs, community resources, etc
  • Maintaining hospice patients' charts and other documentation

Qualifications
The chosen candidate must possess:
  • 2 years' experience as a Social Worker
  • Computer proficiency to include the ability to chart electronically
  • Fluency in Spanish
  • Reliable transportation and the willingness/ability to cover a 60-mile radius on a regular basis
We also prefer:
  • Master's in Social Work
  • Licensed Social Workers (LCSW, LICSW)
  • Hospice experience
  • Social services experience in a healthcare setting or agency
  • Experience working with the geriatric population in a medical/behavioral capacity
